Help! Tankomizer spike came out when preparing new tanko..

Did I do something wrong? I was preparing a new tanko...and when I removed the tanko after inserting it to punch out the "chad", the spike thing came out along with what I presume is some wiking material...
Did I do something wrong with what is a seemingly fool proof procedure? Is this fixable?
So sad..I broke the golden rule and only ordered one tankomizer
Back to my cartomizers for now, and dripping of course...

Also, for the experienced...would unrecognized leaking and/or flooding over an extended period of time cause a tankomizer to be susceptible to this? I believe one of my tanks was leaking all day yesterday and I didn't realize it...
just for future reference and in hopes that this isn't something I have to be concerned about as an ongoing problem with the tankos...

Make sure that when you pull the tank off of the tanko, that you twist the tank first. That will loosen the seal and aid with tank removal.

Replacements arrived and I am happily vaping with my tanko's again. Definitely my preferred method
